Missouri ACP COVID Waiver and Health Acknowledgement
Please make sure that you have reviewed any advisories or restrictions that may be in place for travel to the Missouri ACP Scientific Meeting. By attending, you are confirming that you and all persons in your party:
• Are not experiencing any of the following symptoms of COVID-19 identified by the CDC at CDC.gov, including:
o Fever or chills
o Cough
o Shortness of breath or difficulty breathing
o Fatigue
o Sore throat
o Muscle or body aches
o Headache
o New loss of taste or smell
o Congestion or runny nose
o Nausea or vomiting
o Diarrhea
• Have not been in unprotected (not wearing a mask/PPE) contact with someone with confirmed or suspected COVID-19 symptoms without completing a 14-day quarantine
• Are not under any self-quarantine orders.
Attendees who cannot confirm all the above criteria must not attend the 2021 Missouri ACP Scientific Meeting.
An inherent risk of exposure to COVID-19 exists in any public place where people are present. COVID-19 is an extremely contagious disease that can lead to severe illness and death. According to the Centers for Disease Control and Prevention, senior citizens and attendees with underlying medical conditions are especially vulnerable.
By attending the 2021 Missouri ACP Scientific Meeting, you voluntarily assume all risks related to exposure to COVID-19 and further acknowledge that you understand and will follow all health and safety protocols established by Missouri ACP and the Margaritaville Lake Resort. Failure to abide by all health and safety protocols implemented for the event is grounds for being asked to leave the event, without refund.

• You can scan your card, save it as an image, then upload the image.
• Take a photo of the card with your phone:
• Email the photo to yourself, then save the image and upload it in the form.
• Complete the online form using your phone and upload the image straight from your phone
